Individuals must be between the ages of 7-18 and can either be referred onto the programme by their GP, School or Social Worker or be self-referred by their parents. An initial full health, fitness, psychological and nutritional assessment will take place, leading to a full FFC plan to be created for each individual. Progression reports will also be provided by the whole of the FFC team each month for the families of each individual on the programme and also for the institution that may have referred them onto the programme. For any questions in regards to the FFC or to join our programme, please contact: info@thereviveproject.co.uk FFC Interactive Platform The interactive platform will provide a private log in for all members of the programme to create their own profile’s whereby they will have full access, 24 hours a day to: a forum to interact with other members of the project and their mentors; videos of home exercise workouts; nutritional key tips; a mood tracker; physical activity log and food diary.
